We’re CareSource, your Grants Pass-based Medicare Advantage provider. If you live anywhere in Jackson or Josephine Counties (or in the towns of Glendale or Azalea in Douglas County), Oregon, we’re here to help you with your healthcare needs – and save you money.

By focusing on our members, keeping our entire workforce local and maintaining low overhead, we’re also able to help you save money over competing plans and Original Medicare.

As your friend and neighbor, we want to help you lower your expenses and ensure you have the finest, most comprehensive care available. Almost all of the doctors and clinics in the area are already CareSource-approved providers. That means you can most likely keep the same provider you’re already using. (If you don’t see your preferred provider in our directory, just call us and we’ll try to get him or her approved.)

CareSource is a local health plan offering comprehensive, affordable care. We are committed to personal, respectful service, and to unfailingly promote member wellness, health education and health care provider involvement.

Hours of Operation: CareSource offices are open from 8 a.m. to 5 p.m., Monday through Friday.
