We’re CareSource, your Grants Pass-based Medicare Advantage provider. If you live anywhere in
Jackson or Josephine Counties (or in the towns of Glendale or Azalea), we’re here to help you with
your healthcare needs – and save you money.

Why Become a CareSource Member?
CareSource was created by local doctors to give southern Oregon’s seniors the great care, outstanding
service, and comprehensive coverage they deserve. By focusing on our members, keeping
our entire workforce local and maintaining low overhead, we’re also able to help our members
save money over competing plans and Original Medicare.
As your friend and neighbor, we want to help you lower your expenses and ensure you have the
finest, most comprehensive care available. You’ll notice that almost all of the doctors and clinics
in the area are already CareSource-approved providers. That means you can most likely keep the
same provider you’re already using. (If you don’t see your preferred provider in our directory, you
can even call us and we’ll try to get him or her approved.)

CareSource is a Medicare Advantage Organization with a Medicare Contract. You must have both Part A and B to enroll, and you must continue to pay your Medicare Part B premium. You must use plan providers except in emergency or urgent care situations and for out-of-area renal dialysis or other services.
